Henna Application Waiver and Release of Liability

I, the undersigned, acknowledge that I am voluntarily receiving a henna tattoo from Rare Bird Art Studio at The Hammock Club. I understand that henna is a natural dye derived from the Lawsonia inermis plant and, while it is generally safe for most people, it may cause an allergic reaction in some individuals.

By signing this waiver, I acknowledge and agree to the following:

1. I am fully aware that henna may cause an allergic reaction, skin irritation, or other unexpected effects.

2. I understand that I am responsible for assessing my own skin sensitivities or allergies before receiving a henna tattoo.

3. I release Rare Bird Art Studio and The Hammock Club, including all artists and representatives, from any liability in the event of an allergic reaction or any other adverse reaction as a result of the henna application.

4. I accept full responsibility for any necessary medical treatment or expenses that may result from an allergic reaction or any other issues arising from the henna tattoo.

5. I affirm that I am a responsible individual who has chosen to receive henna at my own risk.
